What’s the Project About?
The “Don’t Forget My Pet” coffee table book is a project to bring together a beautiful collection of dog photographs and stories. Each page will showcase stunning images taken by Helen Green, accompanied by heartfelt stories shared by the owners. Our goal is to create a keepsake that dog lovers will cherish, while also raising funds to support pets in need through Don't Forget My Pet Foundation Ltd.

How to apply:
Dog owners are invited to apply for their beloved pets to be featured in the book by completing our contact form. Once we have received your application, we will send you a questionnaire about your dog and our full price list. If your application is successful, we will let you know by email. Once you have received an email, and make a $55 donation to Don't Forget My Pet Foundation, owners can secure a dedicated page for their dog. Helen Green will photograph each dog, capturing their unique personality and spirit, while owners provide stories about their dogs to be included in the book. This collaboration ensures that every photo and story is a unique celebration of the dogs that bring joy to our lives.

what's included:
Once your application is approved, and you have made your $55 donation, you will receive the following:
- A guaranteed page in the photo book for one or two dogs.
- A photo session for up to two dogs from the same household.
- A sneak peek preview image on Helen Green’s Facebook page.
- A purchase appointment conducted online for participants to select their favourite images, with 5% of any sales from this project donated to Don't Forget My Pet Foundation by Helen Green.

By applying to have your dog featured in the “Don’t Forget My Pet” coffee table book, you are not only honouring your furry friend but also contributing to a cause that helps pets in need. Together, we can create a beautiful tribute to the dogs that touch our hearts and make a difference in the lives of many.
Terms and Conditions
1. Selection Process: Participants in the book project will be selected after completing the application questionnaire. Once approved, a contract will need to be signed, and an appointment will be made for your photo session.
2. Donation: A donation of $55 will need to be made directly to Don’t Forget My Pet Foundation, and a copy of your receipt is to be provided to Helen Green prior to your photo session. Donations made to Don’t Forget My Pet Foundation are tax-deductible, so make sure you keep a copy of your receipt. Your donation secures a page in the coffee table book for one or two pets.
3. Use of Information: Information provided in your questionnaire may be used in the book and will be available publicly, so only include information that you are happy to share.
4. Photo Selection: Photos in the book will be photographed and selected by Helen Green.
5. Appointment and Cancellation: If your photo session cannot be completed within four weeks of the original appointment date, your place in the book will be forfeited, with no opportunity for a refund. Photo sessions will take place in the Geelong region.
6. Liability Release: By participating in the book project, applicants agree to release and hold harmless Don’t Forget My Pet Foundation Ltd and Helen Green and each of their respective officers, directors, representatives, volunteers, and agents (the Released Parties) from and against any claims of any nature arising out of participation in the book project.
7. Modifications and Termination: The organizers reserve the right to modify or terminate the terms and conditions at any time. Participants will be notified of any changes.
