The "Don’t Forget My Pet" coffee table book is a unique collection of stunning dog portraits and touching stories, captured by renowned photographer Helen Green. Each page will showcase a different dog, celebrating the special bond between pets and their people.

- Get Notified: We’ll send you an email with all the details and the application form before the project is live. Expect an email to be sent from 13th February.
- Apply Early: Be among the first to submit your application and secure a spot for your dog in the book.

Terms and Conditions
1. Selection Process: Participants in the book project will be selected after completing the application questionnaire. Once approved, a contract will need to be signed, and an appointment will be made for your photo session.
2. Donation: A donation of $55 will need to be made directly to Don’t Forget My Pet Foundation, and a copy of your receipt is to be provided to Helen Green prior to your photo session. Donations made to Don’t Forget My Pet Foundation are tax-deductible, so make sure you keep a copy of your receipt. Your donation secures a page in the coffee table book for one or two pets.
3. Use of Information: Information provided in your questionnaire may be used in the book and will be available publicly, so only include information that you are happy to share.
4. Photo Selection: Photos in the book will be photographed and selected by Helen Green.
5. Appointment and Cancellation: If your photo session cannot be completed within four weeks of the original appointment date, your place in the book will be forfeited, with no opportunity for a refund. Photo sessions will take place in the Geelong region.
6. Liability Release: By participating in the book project, applicants agree to release and hold harmless Don’t Forget My Pet Foundation Ltd and Helen Green and each of their respective officers, directors, representatives, volunteers, and agents (the Released Parties) from and against any claims of any nature arising out of participation in the book project.
7. Modifications and Termination: The organizers reserve the right to modify or terminate the terms and conditions at any time. Participants will be notified of any changes.
